We partner with BJC Health System to provide accessible, affordable, and specialized post-discharge medication support for high-risk patients with serious chronic conditions to reduce admissions and readmissions.
LIV Recovery Sober Living
We partner with LIV Recovery Sober Living to give their residents personalized medication support services and help them stay on a path to a healthier lifestyle by making sure they know the important role their medication plays in sobriety and helping them navigate chronic conditions in addition to managing their recovery efforts.
Operation Pathways
We partner with Operation Pathways by having pharmacists go to residential apartment buildings and implementing in-person specialized medication support services with patients with chronic and complex conditions. It allows patients to receive pharmacy care without leaving their building.
